  • We begin with an initial conversation to gather background information and review referral materials. This helps our team understand care needs, goals, and potential program alignment.

  • Clinical documentation is reviewed to assess medical stability, functional abilities, and support requirements. This step ensures services can be safely and effectively provided.

  • Based on the evaluation, we determine which residential setting and level of support may be the best fit. Placement decisions are made with long-term success and safety in mind.

  • Once accepted, our team coordinates admission timing and transition details. We work collaboratively to support a smooth and well-prepared move into the program.

  • Length of stay is determined by the treatment team based on individual goals, progress, and functional outcomes. This is reviewed at least annually and documented through updated physician prescriptions.
